Application for Phlebotomy Technician Program

Please set up a profile and complete this form for our Fall 2020 Phlebotomy Technician Program.  This class is limited to 10 students and additional steps/documents are required before acceptance to the program. 
These steps include emailing the following to  
1.  Resume or Job History 
2.  1 page Letter of Interest explaining (1) why you would be a good candidate for this program and (2) how you hope to use an industry credential in your future career plans. 
3.  Copy of High School Diploma or Equivalent
4.  One of the following Assessment Documents
a.  Copy of WorkKeys Career Readiness Certificate (CRC) taken within the last 12 months.  Minimum of Silver certficiate required for consideration.  The CRC covers the subject areas of Workplace Documents, Applied Math and Graphics Literacy.  You may visit for more information regarding these three tests. These tests are administered at CVCC at a cost of $45. Contact the Workforce Office to schedule an appointment for these three tests (dates pending at this time).  Testing session is approximately 4 hours.
b.  Copy of TABE scores – must have tested within the past twelve months. Minimum scores, Math Level 3 & Reading Level 4, are required. Contact Adult Continuing Education at 434-528-6494 to schedule a testing date. This service has no fee.
c.  Currently have a degree or are attending a college or university. Please submit an unofficial transcript with completed acceptable college level Math and English Composition courses with C or better grade earned within the past twenty-four months.
Criminal Background Check

All Phlebotomy Technician Program applicants will be required to submit a Criminal Background Check with the Virginia State Police via a Workforce Representative. These requests will be processed through the Virginia State Police. The Office of Workforce Development is located at Central Virginia Community College’s Main Campus, Merritt Hall, Room 5108 or by calling 434-832-7607 or The Criminal History Background Check results must be received by the Office of Workforce Development prior to an official acceptance into the Phlebotomy Program can be finalized.

The Phlebotomy Technician applicant is responsible for the $20.00 charge which is payble to the Virginia State Police by Money Order only. This money order is turned into the Office of Workforce Development. Please bring a State issued ID or a State issued driver’s license. Please make an appointment by calling 434-832-7607 for this process to be completed.

Drug/Nicotine Screening

All Phlebotomy Technician Program applicants will be required to have a nine drug screen urine test that INCLUDES nicotine.

Appointments are not needed. Walk-ins are accepted any time between 9:00 – 5:00, Monday – Friday. (Closed Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day)

The student is reponsible for the fee of $60.00 (cash) or $62.50 (credit/debit card).

  • Flu Immunization - $35.00
  • Titers – Free of charge to CVCC PBT students

The nine drug screen and nicotine urine screen results must be included with your application materials. The drug screening results must be emaled to directly from Blue Ridge Scientific or HealthWorks.
